How does developmental care influence motor development in preterm born children

Samenvatting

Premature born infants and the external influences on their motor development have a relation which is not yet completely understood. Early interventions which aim at improvement of motor functions are diverse and with various outcomes, which are not necessarily reliable, due to the fact that maybe only one study has been done on it. Most literature stresses that early intervention is necessary, but there is no complete agreement on the optimal methods and strategies used. In this systematic rewiev we will try and give as much as possible a broad outcome of interventions and their supposed effects, by covering a wide base of literature, and critically reviewing articles concerning interventions which improve motor development in preterm born infants. The role of the physiotherapist is especially taken into concern, with the aim to relate evidence based physiotherapeutic interventions within this field. Research was done within the following databases: Pedro, PubMed, AMED, Picarta, articles which could be used. 18 of Cochrane library, Paediatric Physical Therapy, NEJM and BMJ. This resulted in 30 these were available to use in full text version. All articles were read, and graded according to a criteria list. The final outcome was 12 articles, which passed all inclusion criteria and were used in the review. About one half of the discussed articles showed that early intervention is beneficial for preterm born infants. The other half showed no positive effect of the intervention, however, no negative effects could be found. Physical therapy is mentioned only in a few articles, and has only in one of them beneficial results. The articles in this systematic review are highly devided in their outcomes, and therefore it cannot be said with certanity that one method is more effective than the other. It needs to be taken into high consideration that all the infants are different, and as a consequence will react differently upon any stimulation. Also the childrens home enviroment, and the parents social status have to be considered. Individualized care and sensetive assessment tools seem to have the best outcomes concerning motor development.
