Patients with Focal Dystonia Benefit from Physiotherapy?

Samenvatting

Which physiotherapeutic interventions are most effective in alleviating pain and improving function in patients with focal dystonia, aged 18 – 90? Summary: Dystonia is a bizarre condition characterized by prolonged muscle contractions, and it took decades to accept that this disorder is due to brain disease. Zetterberg et al. (2008) conclude that there is limited literature on the effectiveness of physiotherapy in the management of focal dystonia. Because of the little amount of studies in this area, physiotherapists are left with little guidance on which symptoms in dystonia respond to what treatment approach, and a physiotherapy program useful for patients with dystonia is needed. The purpose of this study is to develop a treatment protocol for physiotherapists based on evidence and reasoning which can be used in daily practice in order to provide effective physiotherapy interventions. Conclusion: Physiotherapy in general decreases pain and improves function in patients with cervical dystonia, musician’s dystonia and writer’s cramp. However, which interventions are most effective remains questionable. Recommendations/suggestion for further research/projects: Perform a randomized controlled trial with the AMC who are planning to do so in order to construct a treatment protocol to be used to train physiotherapists across the Netherlands to treat patients with cervical dystonia.
