Doel
Alle cliënten met aspecifieke lage rugklachten worden op dezelfde manier begeleid door de instructeurs van OFW middels een protocol gericht op deze groep cliënten.

Achtergrond
Bij Olympia Fitnesswereld is te weinig kennis aanwezig omtrent aspecifieke lage rugklachten. Dit heeft als gevolg dat de begeleiding van deze groep cliënten niet optimaal is. OFW heeft de opleiding Fysiotherapie te Fontys Paramedische Hogeschool te Eindhoven benaderd om een protocol op te stellen gericht op het begeleiden van cliënten met aspecifieke lage rugklachten.

Methode
• Literatuurstudie
Literatuurstudie was gericht op oefeningen en trainingsprincipes die lage rugklachten kunnen verminderen. De databases van Medline, Cochrane, Cinahl, Doconline, Picarta, Pedro, KNGF en het NOC-NSF zijn geraadpleegd op internet. Mede is gebruik gemaakt van de Mediatheek op de Fontys Paramedische Hogeschool te Eindhoven.

• In -en exlusiecriteria:
De gevonden literatuur is aan de volgende in - en exclusie criteria getoetst:
Inclusiecriteria:
- Inhoudelijke overeenkomst met het project.
- Publicatie van het artikel na 1990.
- Onderzochte patiëntenpopulatie groter dan 10 personen.
- Gebruik gemaakt van trainingsapparaten en / of oefentherapie.
- Geschreven in de Nederlandse of Engelse taal.
- Het een Randomized Clinical Trial (RCT) is.
Exclusiecriteria:
- Het een pilot-study betreft.

Criteria bij deze beoordeling:
De gevonden artikelen moesten aan alle inclusiecriteria voldoen en niet aan de exlusiecriteria.

• Methodologische beoordeling:
Met behulp van het beoordelingsformulier voor RCT's is de methodologische kwaliteit beoordeeld.

Criteria bij deze beoordeling:
Minimaal 5 punten positief.







• Intergratie tot protocol
De getoetste literatuur is bestudeerd. De gebruikte oefeningen en trainingsprincipes zijn vergeleken en samengevoegd tot een protocol. Het protocol is getoetst op de volgende eisen:
-contra-indicaties beschreven m.b.t. de doelgroep.
-de juiste trainingsprincipes gehanteerd.
-duidelijk omschreven en gebruiksvriendelijk.
-toepasbaar bij OFW.
-gericht op de begeleiding van cliënten met aspecifieke lage rugklachten

• Intergratie protocol bij Olympia Fitnesswereld
Olympia Fitnesswereld is geraadpleegd om het protocol te kunnen integreren. OFW heeft, aan de hand van eerdere ervaring met het integreren van protocollen, een aantal punten opgesteld waarmee het protocol in het bedrijf geïntegreerd moet worden.

Resultaten
Tijdens het literatuuronderzoek zijn 30 artikelen gevonden. Na toetsing aan de in -en exclusiecriteria en methodologische kwaliteit bleven er 8 artikelen over. Na data-extractie is gebleken dat er weinig wetenschappelijk onderbouwde oefeningen en trainingsprincipes zijn bij aspecifieke lage rugklachten. Daarentegen blijkt uit de onderzoeken in de artikelen dat oefenprogramma's de pijnsensatie en de beperkingen verminderen.

Conclusie
Uit dit onderzoek blijkt dat fitness een positief effect heeft op pijn en beperking bij mensen met aspecifieke lage rugklachten. Er is een groot verschil waargenomen in het aangeboden fitnessprogramma van de RCT's. Sommige RCT's beschreven het oefenprogramma niet. Het is mogelijk geweest om het protocol op te stellen met de gebruikte RCT's. Hierin werden, met uitzondering van één oefening, grondoefeningen gebruikt welke niet conform waren aan de eisen van de opdrachtgever, omdat deze een protocol wilde bestaande uit oefeningen met apparaten die bij OFW aanwezig zijn. Na overleg met de opdrachtgever zijn de oefeningen en trainingsprincipes uit de RCT's gecombineerd tot een protocol.
De doelstelling van het project is nog niet behaald vanwege het feit dat het protocol niet bestaat uit oefeningen met apparaten die bij OFW aanwezig zijn. Wel is het protocol wetenschappelijk onderbouwd en gericht op het trainen van cliënten met aspecifieke lage rugklachten. De instructeurs kunnen door de presentatie en demonstratie eenduidig met het protocol gaan werken. Tevens blijft er contact van deze afstudeergroep met de opdrachtgever voor feedback.

Objective
All clients with nonspecific low back complaints get the same support by the instructors of OFW with the use of a protocol focussed on these kind of clients.

Background
OFW has a lack of knowledge concerning nonspecific low back pain. This is why the support of this group of people isn't as optimal as it could be. OFW approached the education Fysicaltherapy at Fontys Paramedische Hogeschool in Eindhoven to make a protocol for the support of clients with nonspecific low back complaints.

Design
• Study for literature
A study of literature has been done in the search for exercises and trainingprinciples that can decrease low back complaints. The databases of Medline, Cochrane, Cinahl, Doconline, Picarta, Pedro, KNGF and NOC-NSF were searched. The library of the Fontys Paramedische Hogeschool te Eindhoven was also used to search literature.
• In and exclusion criteria
The literature that was found is criticized on the following criteria:
Inclusioncriteria:
- Internal resemblance with this project
- Publication after 1990
- Researched patientspopulation more than 10 persons
- The use of trainingsequipment and/or exercise therapy
- Written in English or Dutch
- It concerns a Randomized Clinical Trial (RCT)
Exclusioncriteria:
- It concerns a pilot-study

Criteria with this criticism:
All articles had to be positive with the inclusioncriteria and be negative with the exclusiecriteria.

• Methodological criticism
With the support of the criticizeform of the methodological quality of an RCT the methodological quality was criticized.

• Intergration to an protocol
The criticized articles are studied. The used exercises and trainingsprinciples are compared with and put together into a protocol. The protocol is criticized on the following demands:
-contra-indication described
-the correct trainingsprinciples described
-clearly described and easy to use
-the protocol to use
-focussed on clients with nonspecific low back complain.

• Intergration protocol with OFW
OFW has been consulted to intergrate the protocol. OFW often works with protocols and that's why they have a lot of experience with working with a protocol. OFW put together a sum of points, with which the protocol has to be intergrated.


Results
During the search for literature 30 articles has been found. After the criticism there were only 8 articles left. After the data-extraction it was obvious that there are very less exercises and trainingprinciples evidence based with nonspecific low back complaints.
The literature produced exercises and traingsprinciples. Few exercises are evidenced based exercises for training of nonspecific low back pain. Only fitnessprogrammes are evidence based for the training of nonspecific low back pain. On the other hand, the researched showed that exercise programmes decrease painsensation and disabilities.

Conclusion
The results showed that exercise programmes decrease the pain and disabilities in people with nonspecific low back complaints. A great difference has been perceived between the fitnessprograms used in the RCTs. Some RCTs didn't describe their fitnessprograms. It has been possible to make a protocol based on the RCTs, but these RCTs used ground exercises except for one exercise. The protocol had to be created for the use at OFW with their fitness equipment. That's why this protocol doesn't satisfy the demands of OFW. After a consult with OFW we decided to use the exercises and trainingprinciples from the RCTs for the protocol.
The purpose of this project has not yet been accomplished because of the fact that the protocol doesn´t exist of exercises on fitness equipment available at OFW. But the protocol is evidence based and focussed on the training of clients with nonspecific low back complaints. Because of the presentation and the demonstration the instructors are able to use the protocol in the same way. There will remain contact between this projectgroup and OFW for feedback.
