Samenvatting

Het onderwerp van dit afstudeerproject betreft echografie als diagnostisch middel binnen de fysiotherapie.
De hoofdvraag luidt daarom als volgt:

"Wat is de meerwaarde van echografie voor de fysiotherapeutische diagnose en behandeling van patiënten met orthopedische klachten binnen de fysiotherapie?"

Het project bestaat uit 4 delen:
Deel I bestaat uit het theoretisch kader, waar de achtergrond informatie in opgenomen is. Deel II bevat een literatuuronderzoek naar wetenschappelijke artikelen met betrekking tot informatie over echografie binnen de fysiotherapie, publicatie vanaf 1987. Deel III bevat een enquête en de hierbij horende enquêteanalyse. Deel IV bevat een interview en verwerking hiervan.

Doelstellingen
Deel I, II, III en IV; Een theoretische verantwoording waarin beschreven staat wat de meerwaarde is van echografie voor de fysiotherapeutische diagnose en behandeling van patiënten met orthopedische klachten binnen de fysiotherapie.
Eindproduct: Een voorlichtingscd-rom voor studenten Fysiotherapie en MBRT waarin duidelijk wordt wat de eventuele mogelijkheden en meerwaardes zijn van echografie binnen de fysiotherapie.

Achtergrond
Naar aanleiding van de ISA module MBRT, zijn de studenten Fysiotherapie geïnteresseerd geraakt in de echografie. Echografie is tevens een nieuwe ontwikkeling binnen de diagnostiek van de fysiotherapie. Na verdieping in dit onderwerp is geconcludeerd dat er binnen de fysiotherapie weinig bekend is over de absolute meerwaarde voor de fysiotherapeut en de patiënt. Er is nog geen onderzoek gedaan naar de invloed van bevindingen uit echografische beelden op het fysiotherapeutische onderzoek en de behandeling. Hoewel echografiegebruik al enkele jaren terug in de wereld van fysiotherapie is geïntroduceerd en er op kleine schaal gebruik van wordt gemaakt, wordt er tijdens de opleiding geen aandacht besteed aan echografie. Vandaar dat dit project zich tevens richt op het informeren van studenten over het gebruik van echografie binnen de fysiotherapie. Dit om studenten Fysiotherapie en MBRT inzicht te geven in deze nieuwe ontwikkeling.
Methode
Deel II; het zoeken in verschillende databases naar literatuur, met betrekking tot informatie over echografie binnen de fysiotherapie. Dit leverde dertien wetenschappelijke artikelen op die voldeden aan de opgestelde inclusiecriteria en exclusiecriteria. De validiteit en betrouwbaarheid van de artikelen werd door twee beoordelaars onafhankelijk geanalyseerd en beoordeeld aan de hand van een specifiek beoordelingsformulier. De bevindingen uit deze analyse werden onderling vergeleken en hieruit is een gezamenlijke conclusie getrokken.
Deel III; voor het opstellen van een enquête is veel inventarisatie gedaan naar de ontwikkeling van een enquête via verschillende internetsites en literatuur. Er is een adressenbestand opgesteld met behulp van school en verschillende leveranciers van echografische apparatuur. De enquête is vervolgens opgesteld, er zijn begeleidende brieven geschreven en de enquête is opgestuurd. Hierna zijn de enquêtes geteld en verwerkt in SPSS en Excel en daarna in het onderzoeksverslag.

Deel IV; Voor het opstellen van het interview is een inventarisatie gedaan naar de ontwikkeling van een interview via literatuur en verschillende internetsites. De opzet van een interview is verwerkt. Hierna is het interview opgesteld en zijn de personen aangeschreven die geïnterviewd moesten worden. De opgenomen interviews en aantekeningen worden meegenomen in de verwerking van de resultaten, er is een samenvatting van de interviews gemaakt. Deze samenvatting is opgenomen in het onderzoeksverslag.
Eindproduct; Videoanimaties met een casus en verschillende pathologieën die met echografie te zien zijn moest ontwikkeld worden. Dit werd gedaan aan de hand van een van te voren opgesteld script. Een grafisch en technisch raamwerk moest ontwikkeld worden voor de CD-ROM, waarin de videoanimaties met de casus en verschillende pathologieën geïntegreerd konden worden.

Resultaten
Deel II; Uit het literatuuronderzoek zijn uiteindelijk na selectie 7 van de 13 artikelen beoordeeld met behulp van verschillende beoordelingslijsten te zien in de data extractie tabel.
Hieruit is naar voren gekomen dat echografie relatief hoog scoort op het gebied van sensibiliteit en specificiteit. Dit maakt het een betrouwbare diagnostische methode om een exactere diagnose van verschillende trauma aan het bewegingsapparaat te maken dan wanneer enkel klinische testen worden gebruikt. De ervaring en interpretatie van de onderzoeker speelt hierbij echter een grote rol.
Deel III; De enquête is naar 37 fysiotherapie praktijken gestuurd die door het hele land hun beroep uitoefenen. De respons op de sluitingsdatum was 19. We kunnen spreken van een goede respons van 51%. Uit de resultaten van de enquête blijkt dat door middel van echografie vele structuren en pathologieën van het bewegingsapparaat te diagnosticeren zijn. De fysiotherapeuten geven aan dat dit een grote aanvulling is ten opzichte van enkel een functieonderzoek. Ook geeft men aan dat tussen de behandelingen in door middel van echografie de vorderingen van de behandeling geanalyseerd kunnen worden en hierdoor kan bijtijds het behandelplan aangepast worden. Tevens is er uit de enquête gebleken dat door middel van het gebruik van echografie er een gerichtere behandeling aan de patiënt kan worden aangeboden. Daarnaast kan een fysiotherapeut door middel van echografie een bevestiging vinden die via het lichamelijk onderzoek al naar voren is gekomen, hierdoor is zekerheid gewaarborgd.
Deel IV; Er is de keuze gemaakt om het interview te beperken tot één persoon. Deze geïnterviewde fysiotherapeut heeft al 11 jaar ervaring met het gebruik van echografie in combinatie met fysiotherapie. Het beeld van het gebruik van echografie door een fysiotherapeut is duidelijker geworden. Hiervoor is overigens geen protocol ontwikkeld. De belangrijkste resultaten uit dit interview zijn dat het duidelijk is geworden dat de toegevoegde waarde van echografie binnen de fysiotherapie vooral zit in het stellen van betere diagnoses, waardoor je adequater kunt behandelen.

Conclusie
Echografie is een betrouwbaar diagnostisch hulpmiddel dat zowel tijdens onderzoek als evaluatiemomenten kan worden gebruikt.
De betrouwbaarheid is echter afhankelijk van de vaardigheid, kundigheid en interpretatiecapaciteit van de onderzoeker.

Er kan een exactere diagnose gesteld worden waardoor het behandelplan goed afgestemd kan worden op de betreffende aandoening en er adequater behandeld wordt.
Door het herstelproces tussentijds te volgen door middel van echografie kan de behandeling bijtijds worden aangescherpt en kan de behandelstrategie stevig worden onderbouwd met argumenten in combinatie met het functieonderzoek.
Als laatste heeft het echografisch onderzoek een informerende en motiverende invloed op de patiënt.


Summary

The subject of this thesis concerns sonography as a diagnostic tool within the physiotherapeutic clinical diagnosis and consists out of four parts.
The main question of this thesis is:

"What is the surplus value of sonography for the physiotherapeutic diagnosis and treatment of patients concerning orthopaedic complaints within the physiotherapy?"

Part II is a literature search of studies for recent evidence (1987 up to 2007) concerning information about the subject sonography in the physiotherapy.
Part III adds a survey and the hereby belonging analysis.
Part IV contains an interview and the processing of these. The last part describes the development of a CD-ROM that encloses: video-animations of a case and several pathologies which are viewed with sonography.

Objectives:
Part I, II, III and IV; These parts form our theoretical findings where the plus-value of sonography for the clinical diagnosis and treatment of patients with orthopaedic disorders within the physiotherapy is being described.
Final product: this part forms an information CD-ROM for Physiotherapy and MBRT student which shows the possibilities and plus-values of sonography within the physiotherapy.

Background
As a result of the ISA module MBRT, the physiotherapy students' interest aroused in sonography. Sonography is a new development within the physiotherapeutic to diagnose muscular skeletal disorders. After research in this subject conclusions were made that little literature is available about the plus-value of sonography for patients and physiotherapist as well.
There has been no research about the influence of findings on sonographic images on the physiotherapeutic diagnosis and treatment.
Although sonography was introduced some years ago within the physiotherapy and already is being used on small scales, no attention is given to this subject during the physiotherapy course. Therefore this project also aims to inform students about the use of sonography within the physiotherapy so that physiotherapy students and MBRT students will be informed about this new development.

Method
Part II: the research in several databases on literature, containing information about sonography within the physiotherapy. By this, thirteen scientific articles were found that satisfied our inclusion and exclusion criteria. The validity and reliability of the articles were analyzed and judged by 2 independent valuers by specific checklists. The separate conclusions from this analysis were being compared and a common conclusion was being drawn.
Part III: to establish the survey, a lot of inventory was made about the developments of a survey by doing research on several internet sites and literature. A list of addresses was made with help of school and several suppliers of sonographic equipment. The survey was made, accompanying letters were written and the survey was send to the addresses. After the surveys were send back they were counted and processed in SPSS and the project.


Part IV: To make the interview an inventory was made about the development of an interview by doing research on several internet sites and literature. The setup of the interview was processed and made. The persons to be interviewed were contacted and the results were processed. A summary is added to the project.
Final product: Video animations of a case and different pathologies that can be diagnosed by sonography were to be developed. A script was made and a graphic and technical frame had to be developed for the CD-ROM in which the video animation of the case and different pathologies could be integrated.

Results
Part II; After the research on literature eventually 7 of the 14 articles were assessed using several assessmentlists wich are shown in the data extraction table. The conclusion out of this assessment is that sonography scores relatively high in sensitivity and specificity. This makes sonography a reliable diagnostic method to make a more precise diagnosis of several trauma concerning the musculoskeletal system than using solely clinical tests. However the experience and interpretation of the investigator plays an important role.
Part III; The survey was send to 37 physiotherapy practices spread throughout the country. The reply on the closing date was 19. The total reply of 51% can be called high.
After analysing the results of the survey, conclusions were made that sonography can diagnose a lot of different structures and pathologies of the musculoskeletal system. All the physiotherapists mention that sonography gives a great surplus value compared to using solely clinical tests. They also mention that sonography can analyse the progress of treatment so that, if necessary, the plan of treatment can be adjusted. Also the survey proved that by using sonography a more specific treatment can be given to the patient. Sonography also gives confirmation of what had been found earlier by clinical tests. Because of this, certainty is being guaranteed.
Part IV; The choice was made to interview one person. This physiotherapist had 11 years of experience in using sonography in combination with physiotherapy. The vision of the use of sonography by a physiotherapist has become more clear.
The most important result out of the interview is that the surplus value of sonography within the physiotherapy mostly consists of the ability to make better diagnosis so that treatment is more adequately.

Conclusion
Sonography is a reliable diagnostic tool that can be used during clinical research and moments of evaluation.
Though, the reliability strongly depends on the skills, experience and interpretation capacity of the investigator.

A more exact diagnose can be made so that the plan of treatment can be coordinated on the concerning disorder so that treatment will be more adequately.
By following the recovery process during the time of treatment by sonography, the treatment can be accentuated and the plan of treatment will be well-founded with arguments in combination with the clinical research.
As last the sonographic research has an informing and motivating influence on the patient.
