Samenvatting

Onderzoeksvraag: Is er uit wetenschappelijk onderbouwde fysiotherapeutische therapievormen voor tension-type headache een fysiotherapeutisch behandelprotocol op te stellen dat toepasbaar is in de praktijk?
Doelstelling: Een behandelprotocol voor tension-type headache bestaande uit fysiotherapeutische therapievormen die wetenschappelijk onderbouwd zijn en dat toepasbaar is in de praktijk en dat in een vervolgonderzoek gebruikt kan worden om de werking van de nektrainer aan te tonen.
Methode: Na het opstellen van de onderzoeksvraag is er gezocht in verschillende databases, waaronder Medline, Pubmed, Cinahl, naar geschikte/bruikbare wetenschappelijke artikelen.
Met behulp van de volgende keywords is er gezocht: Tension Type Headache, treatment, physiotherapy, pathology, massage, exercise therapy, medicine, relaxation, biofeedback, autogenic training, stretching, physical, therapy. Boliances die gebruikt zijn, zijn 'AND', 'OR' en 'NOT'.
Uit de bestudeerde literatuur is het theoretisch kader opgesteld.
Voor het beoordelen van de methodologische kwaliteit van de gevonden literatuur is gebruik gemaakt van een beoordelingsformulier die de onderzoeksgroep aan de hand van formulier II, de beoordeling van Randomised Controlled Trials, van de DUTCH COCHRANE CENTRE heeft opgesteld. Op de artikelen, die geïncludeerd werden in het onderzoek, is data-extractie toegepast aan de hand van een door de onderzoeksgroep opgesteld data-extractie formulier.
Resultaten: 84 abstracts van artikelen zijn gevonden in de verschillende medische databases. 12 artikelen werden vervolgens opgehaald of opgevraagd. Van deze 12 artikelen zijn er 3 systematic reviews en 9 RCT's. De 3 systematic reviews zijn niet methodologisch beoordeeld, maar zijn gebruikt als hulpmiddel bij het zoeken naar verdere literatuur. Aan de hand hiervan zijn er nog 8 RCT's gevonden.
Totaal zijn er bij de twee zoekacties 20 artikelen gevonden, waarvan er na het toepassen van de in- en exclusiecriteria 17 artikelen zijn meegaan naar de methodologische beoordeling. Er zijn 9 artikelen geselecteerd voor de data-extractie, omdat deze een fysiotherapeutische behandelmethode voor tension-type headache beschreven, die bruikbaar zou kunnen zijn in het fysiotherapeutisch behandelprotocol. Voor de opzet van het behandelprotocol is gebruik gemaakt van een methodologisch voldoende beoordeeld artikel (Torelli et al. (17)).
Aan de hand van dit artikel is er gezocht naar een wetenschappelijke onderbouwing van elk van de, in dit artikel gebruikte, behandelmethoden. Het betreft hier de volgende behandelmethoden: progressieve spierrelaxatie volgens Bernstein en Borkovec, massage, relaxatie d.m.v. rekken, en een thuisoefening met relaxatie
Conclusie: Het artikel van Torelli et al. (17), waarin een behandelprogramma voor tension-type headache beschreven staat, is als basis voor het protocol gebruikt. De therapievormen die uit het behandelprogramma zijn gehaald zijn onderbouwd met informatie uit de andere artikelen van mindere kwaliteit. De therapieën die uiteindelijk zijn betrokken in het protocol zijn massage, progressieve spierrelaxatie en rekken, welke worden aangevuld met een thuisoefening.
Uit de resultaten is gebleken dat de progressieve spierrelaxatie, volgens Bernstein en Borkovec, het meest effect heeft op de tension-type headache patienten. Verder is naar voren gekomen dat massage een positieve invloed kan hebben op de frequentie en de duur van tension-type headache. Het rekken van spieren met als doel relaxatie heeft in het verleden duidelijke verbeteringen met betrekking tot spierspanning gegeven. Dit is echter niet specifiek onderzocht bij tension-type headache. Ook is aangetoond dat huiswerkoefeningen volgens de methode Bernstein en Borkovec effectief zijn bij de behandeling van tension-type headache.
De resultaten gebruikt voor het vormen van het protocol komen uit methodologisch 'twijfelachtig voldoende' artikelen. Bij gebrek aan methodologisch betere artikelen is er uiteindelijk een 'best evidenced' fysiotherapeutisch protocol opgesteld voor tension-type headache. Dit protocol is niet toepasbaar in de fysiotherapeutische praktijk, omdat deze meer tijd in beslag dan de standaard tijd per zitting, wel is het toepasbaar in een vervolgonderzoek. Er kan dus geconcludeerd worden dat er meer onderzoek, van methodologisch betere kwaliteit, verricht moet worden naar de effectiviteit van fysiotherapeutische therapievormen bij tension-type headache.


Summary
Study design: literature research
Primary question: Is it possible to make a physicaltherapeutic treatmentprotocol out of evidence based physicaltherapeutic therapies in Tension-Type Headache which can be used in physical therapy practice?
Objectives: A treatmentprotocol for tension-type headache consisting of physicaltherapeutic therapies which are evidence based, and can be used in physical therapy practice. The protocol should be usefull for conducting further research to verify the application of the 'nektrainer'.
Method: After formulating the definite main question competent evidence-based articles have been searched in various databases, including Medline, Pubmed, Cinahl.
Using the following keywords the research is conducted: Tension Type Headache, treatment, physiotherapy, pathology, massage, exercise therapy, medicine, relaxation, biofeedback, autogenic training, stretching, physical, therapy. Boliances used, 'AND', 'OR' en 'NOT'.
The theoretical frame was constructed by studying relevant literature.
The methodological quality of the articles was determined by using an appraisalform made by reseachers of this study deduced from the Cochrane checklist for the screening of a Randomised Controlled Trial (RCT) from the Dutch Cochrane Centre.
The data included in this study were extracted, using a checklist for data-extraction, made by the researchers of this study.
Results: 84 abstracts were found through several different medical databases. Subsequently 12 articles were collected. Of which 3 systematic reviews and 9 Randomised Controlled Trials (RCT). The methodological quality of the systematic reviews is not determined, these are only used as rescource for further search of literature. 8 articles were found in the second search.
In total 20 articles were found, 17 remained after using the in- and exclusioncriteria and were methodological examined.
9 articles were selected for data-extraction, because they described a physicaltherapeutic treatmentmethod in tension-type headache, which could be useful for the protocol.
For the scheme of the protocol the article of Torelli (17) is used because of it's sufficient methodological quality.
Scientific support of each of the treatmentmethods, used in this article. Concerning the following treatmentmethods: Progressive Muscle Relaxation according to the method of Bernstein and Borkovec, massage, strechting for muscle relaxation and a home-based exercise.
Discussie/Conclusie: The article from Torelli et al. (17), in which a treatment program for tension-type headache is mentioned, is used as a basic for the protocol. The therapies substracted from the treatment program are supported with information from articles that had a less scientific value. The therapies that finaly were included in the protocol consisted of massage, progressieve muscle relaxation, streching for musclerelaxation and a home-based excersice.
Progressive relaxation according to Bernstein and Borkovec was the most researched therapy in tension-type headache patients, according to the results. The results also showed that massage had a positive effect on the frequency en duration of tension-type headache. In the past muscle stretching with the intention to relax showed a clear improvement in muscletension. Although this is not specificly investigated in tension-type headache. It is also proved that a home-based exercise program according to Bernstein and Borkovec is effective in the treatment of tension-type headache.
The result used in forming the protocol came from articles with a poor methodological quality. With a lack of methodological beter articles there finaly is formed a 'best evidenced' physiotherapeutic protocol for tension-type headache. This protocol is not usefull in the physiotherapeutic practice because it takes more time than a basic session, even so it is usefull for a further study.
So it can be concluded that more research from a beter methodological quality needs to be done for the effectiveness of physical therapeutic therapies in tension-type headache.
