Samenvatting

Niet aangeboren hersenletsel is een aandoening waar veel muziektherapeuten mee te maken hebben. Mensen komen na een CVA of hersentumor, maar ook na ongeval of ziekten als Alzheimer en Parkinson met hersenletsel terecht in een revalidatiecentrum of verpleeghuis. Deze patiënten worden met tal van klachten en problemen opgenomen. Een deel van deze klachten is tijdelijk en herstelbaar, maar een groot deel is blijvend. Deze patiënten krijgen veel te maken met acceptatie- en verwerkingsproblematiek. Dat is één van de redenen, waarom ze worden aangemeld bij de muziektherapie. Daarnaast worden ze behandeld voor psychische, cognitieve en soms ook lichamelijke problematiek. Met welke klachten de patiënt precies te maken krijgt, hangt voor een groot deel af van de locatie van het hersenletsel.

In de hersenen zijn veel verschillende locaties, die elk hun eigen functie hebben. Veel gebieden werken samen en voor de meeste functies geldt, dat niet één enkel hersengebied daar verantwoordelijk voor is. Toch kun je aan de hand van de locatie wel iets zeggen over de stoornis die ontstaat. Schade aan de motorische cortex levert problemen op in de motoriek. De patiënt is verlamd of spastisch. Schade aan de visuele cortex zorgt voor visuele problemen en letsel aan het gebied van Broca levert een afasie op.

Muziek wordt ook in de hersenen verwerkt. Er blijkt niet één enkel 'muziekcentrum' te bestaan. Muziek bestaat uit verschillende facetten, die elk weer in een ander gebied verwerkt worden. Daarnaast gebruik je veel verschillende functies bij het maken en beluisteren van muziek. Elk van die functies kan ook beschadigd raken, waardoor verschillende muzikale stoornissen kunnen ontstaan. Hebben deze muzikale stoornissen consequenties voor de muziektherapeutische behandeling? Ja en nee. Hersenletsel kan ook op muzikaal gebied stoornissen opleveren en de locatie van het hersenletsel bepaalt hoe die stoornis er uit ziet. Als muziektherapeut zul je bepaalde werkvormen wel of niet doen, afhankelijk van die stoornis. Er is geen leidraad: bij deze stoornis doe je dit en bij die stoornis doe je dat. Maar het blijkt in de praktijk, dat muziektherapeuten, uitgaande van de individuele patiënt, toch wel dezelfde oplossingen hebben bij bepaalde stoornissen. Heeft iedereen evenveel last van deze muzikale stoornissen? Nee. Het blijkt dat musici (of mensen die muzikaal actief zijn) meer last hebben van de muzikale stoornissen. Dit heeft drie redenen. Bepaalde hersengebieden worden door musici meer getraind. Het hangt van het instrument af, welke gebieden dit zijn. Schade aan die gebieden levert bij musici grotere problemen op. De musicus ervaart de stoornis eerder en sterker dan niet-musici. De stoornis invalideert meer bij musici en de focus van de musicus ligt meer op de muzikale vaardigheden.



Consequences of the location of brain injury in music therapy for patients with non-congenital brain damage

Non-congenital of acquired brain damage is a disorder that many music therapists have to face. People arrive in a rehabilitation centre or nursing home with brain damage after a stroke or brain tumour, but also after an accident of diseases such as Alzheimer's and Parkinson's. These patients are admitted with numerous complaints and problems. A part of these complaints is temporarily and reparable, but a large part is permanent. These patients will face acceptance and processing problems. That is one of the reasons, for which they are presented at the music therapy. Moreover they are treated for mental, cognitive and sometimes also physical problems. What exact symptoms the patient has to deal depends to a large extent on the location of the brain damage.

In the brain are many several locations, each with its own function. Many areas work together and to the most of functions applies, that not a single brain region is responsible for that. Yet you can say something by means of the location, concerning the impairment which arises. Damage to the motor cortex creates problems in locomotion. The patient has paralysed or spastic. Damage to the visual cortex creates visual problems and injury to Broca's area provides an aphasia.

Music is also processed in the brain. There is not a single 'music centre'. Music exists from several facets, which are processed each in another area. Moreover use you many several functions at making and listening to music. Each of these functions can also be damaged and several musical disorders may occur. Do these musical impairments have consequences for the music therapeutic treatment? Yes and no. Brain injury can also cause disturbances in musical area and the location of the brain disorder affects how it looks. As a music therapist you shall do or not certain methods, depending on the disorder. There is no guiding principle: at this impairment you add this and at these impairments you do that. But it proves to be in practice, which music therapists, judging by the individual patient, nevertheless the same solutions have at certain impairments. Has anyone experienced the same problems of these musical impairments? No. It seems that musicians (or people who are musical active) are more affected by the musical disorders. This has three reasons. Certain brain areas are more trained by musicians. It depends on the instrument, which areas these are. Damage to these areas provides increased problems in musicians. The musician experiences the disorder earlier and stronger than non-musicians. The impairment invalidates more at musicians and the focus of the musician lies more on the musical skills.
