Een goede transmurale overdracht is belangrijk om de continuïteit van zorg te waarborgen. De ontslagbrief arriveert echter vaak pas laat of is niet compleet. Bovendien wordt de patiënt nauwelijks betrokken bij zijn of haar eigen overdracht. De risico’s van een onvolledige overdracht kunnen groot zijn. Meer dan de helft van de vermijdbare complicaties die plaatsvinden na ontslag, is toe te schrijven aan slechte communicatie tussen de verschillende zorgverleners in het ziekenhuis en de eerste lijn. Door bepaalde aanpassingen komt de transmurale overdracht centraler te staan tijdens de ziekenhuisopname.
