Summary

This report gives a description of the development of a sports-referral system and an implementation plan for this system. The target group for which it was developed are stroke patients on the policlinical rehabilitation department in the Sint Maartens Gasthuis (SMG).
The assignment to develope this product came from the physiotherapy group unit adults of the policlinical rehabilitation department in the SMG in Venlo. The main purpose of the sports-referral system and the implementation plan is to promote durable participation of stroke patients in sport and exercise activities in their own area of living.

For the development of the sports-referral system the manual 'Handboek Revalidatie en Sport' was used as a guideline. To be able to refer properly there has been attempted to determine the level of stroke patients. This has mainly found form via the 'WCN observationlist'. A literature search has also been attempted to determine suitable sportsactivities. There is little literature known about the advantages of specific sportsactivities for stroke patients. The division of sports is thereby mainly subjectively defined and the authors went out from the main effects that take place in health (physical and social) by physical activities.
The physical advantages differ from improved physical condition and muscle strength to a decreased risk of early death. It also decreases the chance of a new stroke. Careful consideration should be taken with vigorous exercise regarding the increased risk of injuries and heart attack.

The greatest problems stroke patients experience, are on recreational activity and social contact level. Sport is an excellent tool to improve social participation. By the participation in sportactivities; especially in groups, patients will more easily participate in social activities .
Research has also shown that exercising reduces the use of medical services.

The sports-referral system consists of a number of parts; a questionnaire in the form of a computer program, an evaluation form and manuals for the use of the parts. By using the computer program the physiotherapist is enabled to give a correct sport/ exercise advice to the patient. The next step for the patient is to contact the sport club on own initiative and start participating in sport and exercise activities. After six weeks and six months the advice is evaluated. Following this evaluation a new advice can be given.

There has been an implementation plan developed to accomplish a good implementation of the sports-referral system. This plan shows the possible limitations and preconditions that the sports-referral system can encounter. The place of the system within the different rehabilitation phases is revealed: first of all the patient is introduced via education with the concept of participating in sports. Following this he or she gets to know sport in an manner. After receiving a sport/ exercise advice the patient practises specific forms of sport at the policlinical rehabilitation department, that have a close connection to the advice. Via this method the patient is well prepared for adjusted sportclubs and exercisegroups in his own neighbourhood/ environment.
Last step is the participation of the chosen sportactivity.
